Ready to begin?
Complete our client form with your child's details. Once it's in, we'll set up your child's file and contact you to book a first appointment, or add your child to our waitlist if we don't have a spot straight away.

- Your funding type (NDIS, Medicare, private health, self-funded or MVA)
- A copy of your referral, if you're using Medicare or MVA funding
